What does HIPAA-secure mean?

HIPAA, or Health Insurance Portability and Accountability Act of 1996, is a United States law developed by the Department of Health and Human Services, which “mandates the adoption of Federal privacy protections for individually identifiable health information”. To put it simply, HIPAA law outlines privacy and security standards designed to protect an individual’s Personal Health Information (PHI). To obtain this HIPAA-secure certification, GoFormz and its employees undertake a variety of advanced security measures, infrastructure protection, and regular data privacy training. What counts as HIPAA data?

HIPAA data includes any shared personal ‘identifiers’, such as an individual’s:

  • Name

  • Address

  • Dates (birth, death, admission, discharge, etc.)

  • Telephone and/or Fax numbers

  • Email address

  • Social Security Number

  • Medical record number

  • Health Plan beneficiary information

  • Account number

  • Certificate/license number

  • Vehicle ID/license plate

  • Device identifiers/Serial numbers

  • Web URL

  • Internet Protocol (IP) address

  • Biometric data (such as fingerprint, voice print, retinal scan)

  • Full face photos and other identifying images

  • Any unique identifying number or code
