Spotlight Form: The Permission Slip
Simplify the distribution and collection of Permission Slips with digital forms
Digital teacher forms can be completed from phones, tablets, and computers, and are available for review and processing as soon as they are submitted. That means teachers no longer must rely on students to thoroughly complete and safely deliver Permission Slips, to and from their homes.
Your digital Permission Slips can be equipped with Signature boxes, to capture guardian permission. These digital Signatures can even be made Required, ensuring the Permission Slip has been authorized prior to submission. You can also add Text fields to capture emergency contact information, student medical concerns, and more.
Using our Public Forms feature, a link to your digital Permission Slips can be emailed to your class’ parent/guardian contact list. Once received, parents and guardians can enter their child’s information and input their signature – no GoFormz account required. After the Permission Slip has been submitted, school admins and teachers can access the form in real time. Teachers can then track the progress of their class’ Permission Slips, and quickly address any missing documents. Public Forms can also be configured with expiration dates, limiting how long a form can be completed by a user outside of your GoFormz account.
